Thank you for your interest in becoming a mentee with our LEAD Men's Ministry or Devoted Women's Ministry! Please fill out the form below and help us get a better understanding of who you are and what your hopes are for a mentoring relationship!

When you are officially paired with a mentor, we will forward this Mentee Intake Form to the mentor assigned to you in order for him or her to know what you would like to receive from this mentorship.

It is required to attend LEAD Men's Ministry or Devoted Women's Ministry in order to be involved in mentorship at Kenosha City Church. All of our mentors are fully trained through these ministries.
